Exceptional Family Member Program The Exceptional Family Member Program (EFMP) is a mandatory enrollment program (MCO P1754.4A) for all Active Duty personnel. The program is designed to provide assistance to service personnel with a family member(s) who has special medical and/or educational needs before, during and after relocation due to Permanent Change of Station (PCS) orders. An exceptional family member is defined as an authorized family member (spouse, child, stepchild, adopted child, foster child, or a dependent parent) residing with the sponsor who may require special medical or educational services based upon a diagnosed physical, intellectual or emotional handicap such as Asthma, Cerebral Palsy, Mental Retardation, Dyslexia, ADD, ADHD, Autism, Oppositional Defiant Disorder, or Depression. Disabilities may range from mild to severe.

Enrollment Information and FAQs

Who is eligible for the Exceptional Family Member Program?
Any authorized family member, enrolled in DEERS who may require special medical or educational services based up on a diagnosed physical, intellectual or emotional need.

How do I enroll in the EFMP?
Ensure that your family member is enrolled in DEERS. Obtain the necessary enrollment forms (DD2792 and/or DD2792-1) from your installation EFMP office or you may download from this website. Be certain that the application is thoroughly completed and signed by all of the required medical / educational providers. Return the completed forms to your installation case worker who will review for accuracy and then forward to HQMC for category assignment.

How long may I be enrolled in the EFMP?
For as long as necessary. Disenrollment from the EFMP requires a letter from a medical doctor stating that services are no longer needed. Disenrollment may be prompted by a change in marital status, a change in dependency status of the enrolled family member or a change in the medical status of the enrolled family member.

What are my responsibilities once I am enrolled?
You are required to update your enrollment every two years by completing the DD2792 and/or DD2792-1 and submitting the form(s) to your case worker.

What does the Tricare ECHO (Extended Care Health Options) program provide?
ECHO provides additional medical coverage from Tricare (conus only) such as: assistive services, durable equipment, expanded in-home medical services and rehabilitation care. More information is available about program benefits and eligibility at: www.tricare.mil/echo. To enroll in ECHO or be screened for eligibility, fax the completed DD2792 and DD2792-1 to: (866) 269-5758.

Are families enrolled in the EFMP able to be stationed overseas?
Overseas assignments for all service members and their dependents require screening. If it is determined that sufficient care for the EFM is NOT available, assignment screeners at HQMC EFMP will work to ensure a duty location where both career and EFM needs are met.

Does the EFMP provide relocation support?
Your EFMP Case Worker at both the losing and gaining installation will assist in the transition of medical and educational care. Ask your local EFMP office for a PCS Needs Assessment sheet upon receipt of orders.

How can EFMP help my Exceptional Family Member with Individual Education Plan (IEP)?
Your case worker will assist you with issues ranging from school services, access to therapies,related resources and provide workshops to help families learn to be their family member’s best advocate.

I am enrolled in the EFMP. Do I rate priority housing?
Priority housing is reserved for category 4 families and is determined by the HQMC EFMP Medical Liaison Officers.

LITTLE LEAGUE BASEBALL CHALLENGER DIVISION
Sign-ups for children with a disability. Open to all High Desert residents ages 5-8 or up to 22 if enrolled in a school program. Call for more info 760-524-3062.

Hook Community Center: V.I.P. Basketball – Ages 12-24
Games will be played at Hook Community Center in Victorville on Fridays starting Jan. 15th from 6-7pm. Practice times will vary. Register before Jan. 7th at 5 pm.$30/6 weeks. Call 760-245-5551 for more information.

Hook Community Center: Family Game Nights
All ages welcome! Enjoy playing various board games and Wii. The 3rd Thursday of the month at Hook Community Center in Victorville. Ages 5 to 11 from 5:30-6:30 and ages 12 to 24 from 6:30-7:30! FREE. For more information, call 760-245-5551.

SPECIAL OLYMPICS - IN THE HIGH DESERT
Starting in February for ages 8 yrs. and up. Saturdays-BASKETBALL and Sunday-GOLF. They are always looking for volunteers! For more information, call Wayne at 1-760-245-9669.
